Chigusa Tsukikage (Garasu no Kamen) - Truthfully, they are not alike personality wise. Appearance wise she looks like she could have been a considerably older version of Kaoru... a very evil looking older version. In addition, both have endured physical scarring that affected the way they lived.

Grencia Mars Elijah Guo Eckener (Cowboy Bebop) - This is perhaps one of the strangest correlations for a character and Kaoru... I found relation in them. Both had something happen to them that resulted in similar events. Gren ended up growing breasts despite being a man. Kaoru went opposite. She had a breast removed due to cancer. Serious issue, when I passed by a picture of Gren the thought just snapped together... I am so odd... Oh, both are kind and caring. He is androgynous, somewhat literally. He is situated in a place, planet, where all the people are of his gender; Kaoru attends an all girl facility. Both die premature deaths, him in an anime, not a manga.

Kaoru Hanabishi (Ai Yori Aoshi) - They have the name Kaoru and have brown hair. His character is kind. He has someone he loves but will not marry her due to certain personal issues that also had left physical scarring.

Hiromi Oka (Aim for the Ace!) - She is a freshman in high school. She is focused on a sport. They have similar hairstyles. They both deal with a loved one who was very influential in their lives dying. They are eighteen at the end of the series they are in.

Jin Munakata (Aim for the Ace!) - Appearance wise, there is not much other than that their hair is dark and is more or less the same length. Like Kaoru, he is not the center of the plot. Nevertheless, he is an important character in the series he is in. They have diseases. They are both highly aware of death coming despite they are in their youth and still full of life. They dedicate their last breaths to living. They both had found love and both had given such love up to avoid their beloved's sufferings. They had gained a relationship with someone from a sport where they were friends outside it and friendly rivals on it. The sport of their choices helped them gain a stronger appetite for living. They only had but a few years of true happiness before their deaths. Kaoru had two. Jin had three.

Kaoru Kamiya (Rurouni Kenshin) - Again, they share the name. She is female, obviously. Like Kaoru, she appears in her series when seventeen. She appears in the first episode. She is caring, kind and cannot cook. She is not the most lady-like of people, or in other words tomboyish. She excels in a sport-like activity. She has a nickname, however it is an insult rather than a title of adoration. She loves the leading male character.

Kaoru Konoe (Gate-Keepers) - Again, they share the name. She is female, obviously. She has short brown hair and is in her first year of high school. She has confidence in what she can do. She has a childhood dream dealing with athletics. She is cheerful but also is determined and hardworking.

Kaoru Nagisa (Neon Genesis) - They look nothing alike, but they share the name Kaoru... Then again, that is a debate. Some write his name as Kaworu. Additionally, he knows a girl with the name Rei. In addition, he is one who seems wise for one so young, open, and even affectionate. He does not really have a gender either so... that somewhat goes in hand with androgynous?

Makoto Kino (Sailor Moon) - The correlation mainly deals with the anime for both. They both have wavy brown hair and greenish eyes. They are taller the main character as well as befriends the main character. Both have a sport they excel in. They are both considered tomboyish. They usually are seen wearing green. Both deal with troubled pasts concerning a man. They are kind, caring, and often are the ones people go to with problems. They stand up for others, in fact, both have been found to save the main female character from others.

Miki Kaoru (Revolutionary Girl Utena) - Okay! Okay! His last name is Kaoru and not his first, I know! Still though... like Kaoru he is intelligent, successful, talented, and kind-hearted. He also is in a grade that does not suit his age. As Kaoru is able to do in the anime, he plays the piano. He is also talented in a specific sport. He is selfless and helpful, like when tutoring those who need help on work. He has a feminine appeal in correlation to Kaoru having a masculine appeal. Oh, and the bangs! They are cut the same way.

Ruka Tsuchiya (Revolutionary Girl Utena) - He disappeared from school after being diagnosed with a terminal illness and was obviously hospitalized before he came back. He was the captain of a sports club. He influenced Juri Arisugawa into fencing and found she was wonderful at it. Kaoru was the one who suggested Rei try basketball. Unlike the main character, he is aware of what is going on with two people, Juri and a girl named Shiori. Kaoru knows what is going on between Rei and Fukiko. He knows Shiori is Juri's weakness like how Kaoru knows Fukiko is Rei's weakness. He, like Kaoru, tries to convince his friend to break away from the bonds. He believes his friend can be the proud, strong woman he knows she is. Kaoru knows how Rei is like when lively and wants her to be happy.

Utena Tenjou (Revolutionary Girl Utena) - She is an androgynous female. She is noble. Girls greatly admire or have crushes on her. The prince thing goes in hand. Both dress in a masculine way. She is loyal and generous. She protects those who are mistreated. She is a leader. She is great at basketball. She wants a guy, not a girl. In addition, in the movie, Utena's bangs are cut the same way.

Yu Sakurakouji (Garasu no Kamen) - I lack in seeing similar personalities here as well. It is his looks. The hairstyle, how his bangs fall... just grow the back out a bit and wha-la!
